Fall 2010 - Late Registration Sign-ups
1. Please register your player at www.eAYSO.org.
2. Mail two (2) signed player applications and $125.00 to:

All player applications accepted after June 17th are considered late and will be placed on a waiting list.
We will make every attempt to place every child on a team for the Fall 2010 season.
Player applications submitted with a Coach or Referee Application will be guaranteed team placement:
1. Coach or referee application are available under AYSO 889 FORMS and Region Documents at http://www.ayso889.org/PageCustom.aspx?id=41&o=218257
2. Clink the link for the appropriate application you are submitting and complete all information requested
3. Click the link for Volunteer Application (Adult)
4. All forms should be signed and mailed with player application to the address above.

Returning Players
If you are a returning player your pre-printed form will be available at registration, or you can update your player application at www.eAyso.org, print and bring (3) signed copies, doctor and emergency contact information including phone numbers, check or cash, parent or legal guardian with photo identification to registration.
Coaches and Referees
If you will be coaching or refereeing for Fall 2010 please complete the following documents and bring (3) copies:

If you are new to Region 889 complete a Volunteer Application form at www.eAyso.org, or if you are a returning coach or referee update your volunteer application at www.eAyso.org and bring (3) signed copies to registration.
