One box will have blank forms

AVCA Waiver and Release Form Fall 2022 

fill out the form (must be legible)
for each child you have playing 
AYSO 889 Aliso Viejo for fall
 (please add your child(s) Date of Birth on the form)
then place forms in the collection box
