AYSO Region 889 Live Scan Information and Registration Instructions

- August 19th, Foxborough Park, 6:00 p.m. - 8:00 p.m. (or as long as needed).
- August 25th, Referee Course, at The Church of Jesus Christ of Latter-day Saints, 7 p.m. - 8 p.m. (or as long as needed).
Dear Volunteers,
Region 889 will be hosting a Live Scan Fingerprinting event just for you on the above dates. All volunteers still needing to get their Live Scans done are invited to get fingerprinted on any of the above dates. Frank Bernal, Region 84 referee and Live Scan Technician/Owner of Confidential Live Scan will be conducting the fingerprinting. Confidential Live Scan is a CalDOJ certified Live Scan fingerprinting provider, authorized to provide fingerprinting services for AYSO. Furthermore, Frank is a retired FBI special agent and the security of your personal information is his number one priority.
In order to get printed, you must register the day prior you wish to get fingerprinted, via one of the two methods below. Walk-ins may be allowed but only as time permits. Priority will be given to those who have pre-registered. You may register via the below link; however, in lieu of registering via the link, you may register by completing one copy of the attached Request for Live Scan Service Form 8016 and mailing it to Confidential Live Scan at the below address. All mailed Form 8016s should be received no later than day prior to the event. Details for completing Form 8016 can be found below. Please only register via one of the two methods.
In order to register online, please click on the below secure Confidential Live Scan link and enter your information as needed by CalDOJ to process your background check. (Your information is encrypted in-transit and at-rest by Google.) By submitting your responses, you acknowledge you have reviewed the attached privacy disclosures, the information provided is true and correct, and you agree to Confidential Live Scan entering the information in the Request for Live Scan Service Form 8016 (Form is required by CalDOJ), which Frank will present to you at the event for your review and signature. You will be provided a copy for your records; therefore, if you registered via the link below, there is no need to complete a hard copy.

**On the day of the event, you will need to present your unexpired California Driver's License or unexpired Other (photo) ID you registered with per CalDOJ, no exceptions please. Upon arrival, please check-in with Frank. Each scan only should take a few minutes. If you tend to have dry or rough hands, please hydrate the day of and day prior to the Event, as well as applying lotion beginning two days before the Event. Please do not apply lotion on the day of the Event.
